Be Ready with the Following Information

When scheduling an appointment, the receptionist will gather information to ensure enough time is allocated for your visit.

If you are a new patient, we will need the following:

  • Insurance Company Name
  • Policy or account number
  • Patient Name
  • Address
  • Social Security Number

Information on your injury:

  • Was it sports related?

Tell us about your condition:

  • Type of ailment/injury
  • When it started
  • Do you have any xrays or MRI films

Once Your Appointment is Scheduled

Please arrive for your scheduled appointment on time. Patients who arrive more than 15 minutes after their scheduled time will be worked into the schedule and seen as soon as possible; but there may be a lengthy wait. If you are unable to keep an appointment, please call us at least 24 hours in advance so we may use those times for other patients.

Cancellation Policy

Please call 24 hours in advance to cancel or reschedule your appointment. Otherwise, we will have to charge you a $25 no show fee.
